Privacy Policy

When you request an appointment, we ask for your first and last name, email, phone number, the type of visit, and your scheduling preferences. That's it.

Your request is emailed to our front desk so a team member can call or email you to schedule your visit. We do not sell your information or share it for marketing.

Our website form is for scheduling only. Please don't include medical history, insurance numbers, or symptoms. We'll go over those with you privately by phone or at your visit. Your office records are covered by our separate Notice of Privacy Practices, available at the front desk.

We don't use advertising or tracking cookies. We count visits in a basic, anonymous way to understand which pages are helpful. The map on our Contact page is provided by Google Maps, which may set its own cookies.

Links to Google Maps, Facebook, and Instagram take you to those sites, which follow their own privacy policies.
